Appointments in Diagnostic Evaluation Series:
Consult appointment: This is a telehealth appointment for caregivers and the clinician to discuss concerns. The clinician will send a link to join the meeting online.
Testing appointment: This is an in-person appointment at the clinic with caregivers and the child present.
Feedback appointment: This is a telehealth appointment for caregivers and the clinician to discuss the results and diagnostic report. The clinician will send a link to join the meeting online.
Important Requirements for your Diagnostic Evaluation:
Prior to the consult appointment you must make sure the Medical History has been completed for your child, there are several pages to complete and the clinician will not be able to evaluate your child without the information. The Medical History is accessed by logging into your Patient Portal selecting Records from the main menu, Medical History, then Pediatric Medical History. Cancellation Policy Reminder:
If your child is going to be absent from sessions for any reason, please notify ICAN as soon as possible. Please include your child's first and last name, date(s) of sessions you wish to cancel, and reason for cancellation, and we’ll work on the opportunity to reschedule your child’s appointment(s).
A cancellation made less than 24 hours in advance will be considered a late cancellation and may be assessed a fee of $10.
A cancellation made less than 1 hours in advance will be considered a no show and may be assessed a fee of $150.
