Evaluation Information
Important Requirements for your Feeding Therapy Evaluation:
Please bring 3 foods your child eats and 3 foods your child doesn’t eat to the evaluation appointment.
Prior to the appointment you must complete the Feeding Therapy Evaluation Questionnaire. The clinician will not be able to evaluate your child without this completed.
Prior to the appointment you must make sure the Medical History has been completed for your child, there are several pages to complete and the clinician will not be able to evaluate your child without the information. The Medical History is accessed by logging into your Patient Portal selecting Records from the main menu, Medical History, then Pediatric Medical History. Cancellation Policy Reminder:
If your child is going to be absent from sessions for any reason, please notify ICAN as soon as possible. Please include your child's first and last name, date(s) of sessions you wish to cancel, and reason for cancellation, and we’ll work on the opportunity to reschedule your child’s appointment(s).
A cancellation made less than 24 hours in advance will be considered a late cancellation and may be assessed a fee of $10.
A cancellation made less than 1 hours in advance will be considered a no show and may be assessed a fee of $150.
